Terms & Conditions

The acceptance of this form, as or on behalf of the student, voluntarily accepts unconditionally and unreservedly full and absolute liability for all risk of injury or damage howsoever incurred, whether by negligence or otherwise as a result of and/or the activities inherent in the tuition of the classes AND AGREE that this acceptance shall constitute a complete waiver, discharge and defence to Encore Performing Arts (QLD) Pty Ltd to any claim, demand or action against Encore Performing Arts (QLD) Pty Ltd for any such damage or injury. I give full authorisation for Encore Performing Arts (QLD) Pty Ltd to use students photos in all marketing materials including, but not limited to: website, internet, promotional advertisement in the form of hard copy materials and social media. I the parent/guardian give full permission for an ambulance to be called in an emergency if I am not able to be contacted.
